Nottinghamshire health boss statement after Valdo Calocane review published

NHS England has published an Independent Mental Health Homicide Review examining the case of Valdo Calocane.

Amanda Sullivan, Chief Executive of NHS Nottingham and Nottinghamshire Integrated Care Board (ICB), said:

“Our thoughts remain with the families of Barnaby, Grace, and Ian, who were killed, and also with Wayne, Sharon, and Marcion, who were injured following the terrible attacks by Valdo Calocane in June 2023.

“We welcome NHS England’s publication of the Independent Homicide Investigation report and fully accept the recommendations. We have developed an action plan to address these. Since the attacks, and also the publication of the Care Quality Commission’s Section 48 Special Review, we have worked extensively to support the delivery of improvements in mental health services in Nottingham and Nottinghamshire.

“Extensive governance arrangements, including oversight from the ICB and regional and national NHS England teams, were set up following the attacks to monitor Nottinghamshire Healthcare NHS Foundation Trust’s response to quality and regulatory concerns. Senior ICB leaders are part of this process, including the Chief Executive and Chief Nurse.

“These arrangements provide scrutiny for operational and quality risks and give wider stakeholders, including specialised commissioning teams within NHS England and the Care Quality Commission, a single forum to support and challenge the progress of improvements as set out in the Trust’s quality improvement and transformation programmes.

“We are committed to safe and high-quality services for our communities, and we will continue to provide support and challenge to Nottinghamshire Healthcare NHS Foundation Trust to ensure sustained improvements in the delivery of safe services.”

A joint action plan with Nottinghamshire Healthcare Trust has been published to address the recommendations of the Independent Homicide Investigation report.

Actions already taken include:

  • The ICB has instigated enhanced surveillance on a broad range of quality concerns and improvement plans.
  • An Improvement Oversight and Assurance Group (IOAG) was set up jointly with NHS England’s Midlands regional team to monitor Nottinghamshire Healthcare NHS Foundation Trust’s response to current quality and regulatory concerns. Senior ICB leaders are part of this process, including the Chief Executive and Chief Nurse.
  • Alongside the IOAG process, the ICB holds regular ‘Safe Now’ meetings with the Trust and uses shared reporting and metrics to assess progress against the more immediate safety challenges.
  • A ‘Safe Now’ dashboard supports immediate actions for safety and improvement, with appropriate escalations. This will become part of routine ward-to-board escalation pathways as it is embedded.
  • ICB representatives attend the Trust’s Improvement Board and associated workstreams to triangulate data and actions.
  • A system risk register has been established to ensure that strategic risks are collectively understood and owned.
  • A patient safety specialist forum has been established to support the implementation and learning of the Patient Safety Incident Response Framework.
