Nottinghamshire Police has completed an internal investigation into a security breach.
This related to its Operation Perth investigation into maternity services at Nottingham University Hospitals NHS Trust.
The security breach was identified through internal audit checks on 20 August 2025 and related to the suspected altering or deleting of information held by Operation Perth.
The matter related to a single member of police staff who was suspended while a meticulous investigation was completed by the force’s Professional Standards Department.
Temporary Deputy Chief Constable Rob Griffin said:
“I am now able to provide an update on the investigation concerning the unauthorised alteration and deletion of information from the Operation Perth investigation system.
“The individual concerned was suspended during the investigation but has since resigned. The investigation, which has now concluded, was complex and thorough, and additional audits were undertaken by the Professional Standards Department to establish the broader context of how and why the information was altered or deleted.
“To clarify, altered or deleted does not mean the permanent loss of information. This refers to single data fields being overwritten, amended, or moved within the system, and all affected information has been fully restored.
“Our robust checking and monitoring processes have enabled us to identify every discrepancy and rectify all alterations and deletions. We are entirely confident in the integrity of the Operation Perth investigation system.
“I can also confirm that no information has been shared outside of the investigation and personal medical records were not affected.
“The outcome of the investigation is that there is no evidence of misconduct. It was determined that the individual did not act dishonestly or maliciously, and there is no evidence of third-party involvement. The alteration of the information was in contravention of the processes we have in Operation Perth and therefore related solely to the unsatisfactory performance of the individual involved.
“Families directly affected have been informed and provided with further details regarding their specific case.
“I recognise the impact that the staff member’s actions may have had on families, and I hope that our swift action and candour in relation to this matter has maintained trust and confidence in Operation Perth.”
Operation Perth is attending Donna Ockenden’s Family Meeting on Saturday 17 January in Nottingham and will speak with affected families about this matter as part of an update on its investigation into maternity services.
